How to Get WaveLyfe Support through Utah Vocational Rehabilitation

Follow these steps to receive job coaching, placement, and ongoing support with WaveLyfe as your Community Rehabilitation Provider (CRP).

Seven Steps to Services

Everything from first contact to successful job stabilization—simple, supportive, and centered on your goals.

Step 1 Applicant contacting Utah VR and gathering ID and documents

Contact Utah VR & Apply

Reach out to your local Utah VR office and complete the application. You can attend a brief orientation and apply online or in person.

  • Bring: ID, disability documentation, and (if applicable) SSI/SSDI benefit proof.
  • Parents/Students (14–21): ask about Pre‑ETS (pre‑employment transition services).

Eligibility Decision

A VR Counselor reviews your application and documentation to decide eligibility. If eligible, you’ll move forward; if there’s a waitlist, they’ll explain your priority category and next steps.

Plan Your Employment Goal (IPE)

With your Counselor, create an Individualized Plan for Employment (IPE) that sets your job goal and needed services (e.g., resume help, interview prep, job coaching, transportation supports).

Choose WaveLyfe as Your CRP

Tell your Counselor you’d like WaveLyfe to provide job development and coaching. VR maintains an approved list of Community Rehabilitation Providers; you have the right to choose among qualified providers.

Your Counselor sends WaveLyfe a referral/authorization so services can begin.

Service Authorization & Kickoff

After authorization, WaveLyfe schedules your kickoff session. We review your strengths, interests, schedule, and accommodations, then build a job search plan aligned to your IPE.

  • Resume & applications, interview coaching, employer outreach
  • Supported or customized employment if needed
  • Coordination with your Counselor and supports (family, school, DSPD, etc.)

Job Placement & On‑the‑Job Support

WaveLyfe works with employers, sets up interviews, and provides coaching. After hire, we help with onboarding, training, accommodations, and stabilization on the job.

Your Counselor stays in the loop; progress notes are shared per VR policy.

Stabilization & Case Closure

When you’re stable in your job for a VR‑defined period and meeting your goal, VR may close your case successfully. If your needs change later, you can re‑contact VR.

Do I need VR approval before WaveLyfe can help?

Yes. Because WaveLyfe serves as a VR‑authorized Community Rehabilitation Provider (CRP), your VR Counselor must refer/authorize services before we begin billed services. You’re welcome to contact us anytime with questions.

What services can WaveLyfe provide through VR?

Typical services include job search & development, resume/interview preparation, employer outreach, supported employment, and on‑the‑job coaching. The exact mix depends on your IPE and VR authorization.

How long does eligibility take?

Timeframes vary by individual and documentation. Your VR office can provide current timelines and whether any waiting list applies.
